Our Supported Living program assists people with developmental disabilities in developing and maintaining independent living skills in their own homes. Each individual has the choice of where to live, who to live with, where to work and how to spend their time.  Our staff provides services that are tailored to individual needs, while emphasizing increased independence and participation in the community. Staff provides a high level of on-going personal care and safety assistance so that individuals can live in their own homes and direct their own lives. We encourage independence in tasks for which the individual has the skill and/or ability.

We offer hourly up to twenty-four hour services to support the needs of each person.  The services that we provide are through a Medicaid reimbursed program

We work together with families, county boards of DD, medical professionals and other agencies to develop the individuals we serve and ultimately change lives.  We offer services for people with any type of challenge, barrier, or need, including medical issues or behaviors that interfere with quality of life.

Autumn Days is dedicated to excellence and puts the clients care and well being first.  Our employees are specially trained for their specific areas of service.  All staff members are screened as mandated by the State of Ohio to assure quality care for the people we serve.
